New Patient Registration

To register as an NHS patient with this practice, you must be resident within the practice area and satisfy the U.K. NHS regulations for entitlement to medical services with respect to nationality, work and residence.
To register with the practice please visit reception to complete the registration forms or print the forms from the links below, fill in and bring to reception. We require both the Cobham form and the NHS form to be completed for each family member, even those under 16. We also require photo ID (i.e. passport, drivers licence) and proof of address (i.e utility bill dated within last 3 months, council tax bill, bank statement).
One of these forms is a medical questionnaire which is required as it can take a considerable amount of time for us to receive your medical records.
Please provide us with all relevant details including a mobile number and next of kin contact details.
Medical treatment is available from the date of registration, our reception team can provide further information if required.
We would also recommend registering for online appointment booking and repeat prescription requests. PLEASE NOTE: Any non-repeat items required should be made in writing to the surgery in the usual way. Additionally, for safety reasons, we do not take medicine requests over the telephone.
